COMMUNITY BASED SUPPORTS

Our well-trained staffs provide personalized Services that provide direct support and assistance for individuals in or out of their residence to achieve and maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. We offer customized training in acquiring, retaining, or improving skills related to activities of daily living, socialization, adaptive living skills, and community integration.

Community Inclusion Services

Sunrise Community Services offer community inclusion programs aimed at developing activities and skill acquisition to support or further integrate community opportunities for individual outside of his or her home. These supports aim to foster independence, autonomy, career exploration and encourage development of a fulfilled life within the individual’s community. Services are person centered and based on likes, dislikes, interests, and aptitude of the person being supported.

Personal Care Services

At Sunrise Community Services, we recognize the importance of providing the supportive care necessary to help our clients maintain their independence and remain safe in their own homes, and our personal care services are designed to do exactly that. Whether assistance is needed a few hours a week or 24/7, we have the ability to customize our services to provide whatever level of support you require. Our personal care services include:
